LifeWeighs Wellness Coaching is pleased to partner with your clinic to support your participation in a year-long diabetes prevention lifestyle change program.
The CDC-recognized Prevent T2 Program emphasizes improving food choices, being physically active for at least 150 minutes each week, and developing skills to identify and overcome barriers to making lifestyle changes.
Your coach will help you learn new skills, encourage you to set and meet goals, and keep you motivated. A close partnership with your coach provides insight, inspiration, structure and accountability.
You will either attend classes at a local site or work with a coach by phone, at times that are convenient for you. During the first 4 months, you will meet weekly and then transition to monthly sessions for the next 6-8 months.
When you are ready to learn more about how you can prevent type 2 diabetes, talk to your provider. If a lifestyle change program is a good fit for you, your provider will get you started on your journey to better health.
